Hyderabad: On the request of several journalists from across the state, the last date for receipt of applications for Medi-claim policy sponsored by the Andhra Pradesh Union of Working Journalists (APUWJ) and the I&PR Department, Government of Andhra Pradesh is extended up to 10th August, 2009.    Since, the first batch of last year policy expires in August, all journalists in State are requested to join the Medi-claim scheme and avail the facility by filling the application form and draw a DD for Rs.780/- in favour of Andhra Pradesh Union of Working Journalists (APUWJ) and send it to the Union Office at Deshoddharaka Bhawan, Fateh Maidan, Bashirbagh, Hyderabad on or before 10th August, 2009.

There are separate applications for renewal and fresh policies. Those who are renewing need not attach their photographs, while the new members should attach two copies of stamp size photo of each family member. We are sending a few application forms to District Units of APUWJ for renewal and fresh policies.  The journalists may also down load application forms from union website “apuwj.org”.
